Before the Safe Drinking Water Act was established in 1974, there was nothing in place to prevent the contamination of clean drinking water when a cross connection was made with a low, or high health hazard plumbing system. A cross connection is a connection of a potable water system (clean & safe drinking water), and non-potable water systems such as boiler lines, chemical treatment lines, and irrigation systems.
A high health hazard connection is water that has the potential to be contaminated by something such as chemicals, sewage or unsanitary water, and other various contaminants. Drinking or coming in contact with this contaminated water may result in various diseases such as Polio, Hepatitis, Typhoid, and others that could result in serious injury or death.
A low health hazard connection is water that is not ascetically pleasing, such as water that has been sitting in a fire sprinkler line that has a rusty look and displeasing taste. This would most likely not pose any major health risks, but still needs to be protected against.
When a backflow prevention device is not installed on the water supply that is directly connected to a line containing these health hazards, this poses many risks for not only your drinking water, but for the municipality and anybody else whose homes or businesses are connected to that water supply as well.

Only a Certified Backflow Preventer Tester is permitted to test a backflow preventer assembly.
The DEP & the municipalities require that all backflow preventing assemblies be tested once annually at minimum. Depending on the application, the municipality, and the health hazard of what the assembly is preventing the backflow of, it may need to be tested up to four times annually.
